1. Go to the Portal

2. On the Welcome page scroll down to the blue Create a Message button and click on it.

(Please note, do not choose a patient before you click the create a message button)

3. Choose a Patient and click on your name

4. Click on the message reason drop down list

5. Click on Well visit Forms

6. Complete the form and click on send.

7. Click Create a Message, again

8. Choose a patient

9. Click on the message reason drop down list

10. Click on 18+ yr old Consent Form

11. Complete the form and click send.
