1. Go to the Portal

2. On the Welcome page of the portal scroll down to the blue Create a Message button and click it.

(Please note, do not choose a patient before you click the create a message button)

3. Choose a Patient and click on their name

4. Click on the message reason drop down list

5. Click on 12 mo Well Visit

6. Complete the form and click on send.
