11 years to 17 years

1. Go to the Portal

2. On the Welcome page scroll down to the blue Create a Message button and click on it.

(Please note, do not choose a patient before you click the create a message button)

3. Choose a Patient and click on their name

4. Click on the message reason drop down list

5. Click on Well visit Forms

6. Complete the form and click on send.

If your child needs a sports physical with the Well Visit:

7. Click Create a Message, again

8. Choose a patient

9. Click on the message reason drop down list

10. Click on GA Sports Physical/cardio

11. Open the link and complete the athlete pages then attach it back to the message as a pdf

12. Complete the cardiac portion of the form and click send.
